This webinar takes a closer look at home dialysis programs in skilled nursing facilities (SNFs) and the importance of innovation. Many of these patients are admitted with multiple comorbidities requiring complex care models to address the need for coordination of care between dialysis and skilled nursing home clinical staff. Lack of collaboration between these two distinct care teams is crucial to decrease the frequent re-hospitalizations found in this population, improve the overall quality of care, and for those patients discharged from the skilled nursing home to the community to reach optimal health status to prevent a turnstile of care. Dialysis professionals need to take the lead in coordinating the care of ESRD patients residing in Skilled Nursing Facilities and ensure the highest level of care is afforded to this unique and often hidden from research and quality measures. - End-Stage Renal Disease patients >65 years of age comprise 52% of annual United States incident dialysis patients.
- Of the incent Medicare dialysis patients, 8,000 initiate dialysis in a skilled nursing facility (SNF) per calendar year.
- In 2013-2015, the mean percentage of all Medicare dialysis patients residing in a SNF during any month was 8.6%, nearly 40,000 patients overall.
- In the subset of dialysis patients aged>75 years old, the mean percentage of patients residing in a SNF during any month is even higher at 14.9%.
- Further, fully one-third of all Medicare dialysis patients live in an SNF in the year before their death.
